Issue with AngularJS: UI Router failing to show the template

I have set up the ui.router with xhtml templates. However, the template that I configured is not showing up.

At this moment, you are using named views but the routing is set up for simple views. Therefore, you should eliminate the name from the ui-view attribute. Both instances of ui-view should be nameless.
