Your Google Home device can only connect to Google servers and doesn't have the ability to connect to a local area network. It doesn't run Actions directly on the device - Actions need to have a URL that can be accessed by Google's servers.
The current recommendation for accessing internal information outside of your LAN is to use a cloud-based resource. A good option for beginners is Firebase Database.
This setup also allows your Assistant (such as the one on your phone) to operate outside of your LAN while still being able to interact with devices within your LAN through the cloud resources.
